Patients who move outside the practice boundary area will be required to register at a GP surgery which covers their new address. If you move outside of our practice area but would like to remain registered as a patient of the surgery, you should contact us in writing within ten days of notifying us of your change of address. The Practice will consider your request and may agree to to allow you to remain on the patient list.
