Travel Advice and Vaccinations

If you need advice about the vaccinations and tablets you might need abroad please collect a travel vaccination form from reception (or download it from this link), complete it and return it (one form per person). The practice nurse will complete the form indicating what treatment you will need. Please collect the form after a week and attend the surgery for treatment as indicated on the form. Please allow at least 8 weeks before your date of departure.

Ideally, you should have your travel vaccinations at least six weeks before your date of departure – to ensure you are fully protected.

As this is not part of the usual NHS service there may be additional charges for some of the vaccinations but you will be told about these before you are offered the treatment.

We need to have some information about your planned destination, etc, before reviewing your records to make sure you are given the correct advice on travel vaccination. You may either collect a form from Reception or print out the form below.

Please complete and return the form to reception and we will work out if you need any vaccinations or anti-malarial tablets. It is your responsibility to contact the surgery to see if you require treatment. You will be told either to make an appointment to see the nurse for treatment or told no treatment is required.

(Please allow at least 72 hours after returning your form before either ringing the surgery after 2pm or calling in)

If your departure is within 7 days or requires complex advice we may ask you to contact Travel Health UK on 01277 200169 to work out what may be required or if anything can be given at short notice.
